Afterschool Forms

ALL STUDENTS: Two Part Enrollment Process

  • In order to complete the two-part enrollment process, all students/parents must submit forms 1 through 5 below after registration.
  • Forms 3-5 can be printed from their portal after registration.
  • You may submit via email, fax or mail. All forms must be submitted before your child can participate in the Afterschool program.
  • Next, click REGISTER HERE, below, to complete enrollment process.
  • Please allow at least 24 hours for processing. There is a one-time non-refundable registration fee of $25 charged to each new account.

We at CDI and CDI After School want every child to be able to participate in our open classes and after school program but we understand that financial hardships are a reality. We offer scholarships and financial assistance based on the availability of funds and the needs of our clients. There are fundraising and volunteer opportunities for parents to help offset costs as well. If you are in need of financial assistance, please click on the link above, CDI Financial Aid Form PDF, to fill out our confidential application.

  • 1. Click on Register Here
  • 2. If you already have an account with Daycare Works, click HERE.
  • 3. If you are a new parent, select your school & category: Monthly schedule, Enrichment Classes or General Registration.
  • 4. If choosing a monthly aftercare option, be sure to select your days of the week and 5pm pick up or 6pm pick up.
  • 5. Click “Enroll” after making your selection and enter all required information before check out.
  • 6. Enter your credit card. Please check your email (and spam/junk mail folder) for receipt.
  • 7. After registration, please login to your account here: https://family.daycareworks.com/login.jsp to add your credit card through the AUTO-PAY tile and your contacts through the PERSONAL tile. Only registered parents and those added to your contact list will be authorized to sign out your children and all will be required to enter their assigned PIN at sign out. PINS can be found under your name by clicking on the PERSONAL tile.
